How FORM came to be

Through her years of experience as an optometrist seeing thousands of patients, it became clear to Dr. Oleksiv that in order to truly help people resolve their eye-issues, a deeper understanding of her patients’ lifestyles and health goals was needed to develop long-term solutions that enhanced their quality of life. Dr. Oleksiv passionately pursued a deeper understanding of the connection between eye conditions and the greater lifestyle trends of the patient, infusing this approach into her practice. Upon the completion of functional medicine training, she decided to open her own practice, dedicated to developing deep, meaningful relationships with the patient in order to truly create sustainable changes to their relationship with mind, body and eyes.

Today FORM, an online functional medicine practice, uniquely leverages the eyes as the window to your overall health, unraveling the intricate connection between chronic eye conditions and broader health considerations. In fact, the name FORM comes from our approach to wellness – Functional and Optometric Restorative Methods. We specialize in personalized, root-cause evaluations – through the lens of functional medicine – that provide you with actionable, sustainable, comprehensive whole-body wellness solutions.

Nadiya Oleksiv, OD, IFMCP

Nadiya is the founder and primary practitioner of FORM Healing. Nadiya received her Bachelor of Science degree from York University, majoring in Kinesiology. While practicing optometry in southern NH for the last 8 years, Nadiya expanded her scope of practice, receiving a certification in functional medicine from the Institute of Functional Medicine (IFM). Originally from Ukraine, I am now proudly rooted in beautiful southern New Hampshire. I’ve always had a passion for helping people see life more clearly! My journey led me to discover the amazing world of optometry at the New England College of Optometry. After completing my Doctor of Optometry degree, I spent eight fulfilling years perfecting my craft in Portsmouth, NH.

During my time in practice, I noticed a common thread among my patients—many eye issues hinted at underlying dysfunction somewhere else in their body. That realization sparked a new passion for the interconnectedness of the human body, and in turn, the desire to expand my practice into functional medicine. During this time, I dug deep to understand the root causes of many health problems and learned how to address a problem at its root – as the most effective way to eliminate it for good, fostering lasting well-being and true healing.

Beyond my professional pursuits, I’m an avid learner and traveler, constantly seeking new experiences that broaden my perspective. Now, armed with certifications in functional medicine and a degree in optometry, my mission is to share insights that help you connect the dots between your eye health and overall wellness. I love making complex health topics simple and understandable. It’s all about empowering you to live your best life.
